Jones Radiology have a duty of care to ensure the safety of all patients and carers while they are in our practice.
We have a formal policy in place for managing transfers, handover and supervision of patients attending a Jones clinic from another facility (e.g. external hospitals, nursing homes etc). Prior to making an appointment for any patient coming from an external facility*, the below form must be completed in entirety.
Upon receiving the completed form, one of our friendly team members will be in touch with appointment options.
This form must be completed by the Referrer or Healthcare professional managing the patient.
*External facility is defined as hospital, nursing home, high care unit within a retirement village. Patients who reside at independent retirement villages can be booked per normal process.
