This form is for medical technology companies that provide joint replacements (e.g. Zimmer Biomet, Medacta) and require access to imaging.
- Submissions must be made by an authorised company representative using a valid business email address.
- All requests will be reviewed and verified before imaging is released.
- Upon completion of the form a support ticket will be generated and assigned to our Referrer Assist team.
- You will receive an email confirmation with the ticket number. This email will come from Automation for Jira <jira@jonesradiology.atlassian.net>. If you do not receive the email within 5 minutes, check your junk folder.
- If you need to provide additional information (e.g. changes in urgency or corrections to patient details), please reply to that email rather than submitting a new form.
